Summary: The
Department of Health and Mental Hygiene "DOHMH" is seeking an
appropriately qualified vendor to collect health survey data for approximately
8,500 adult New Yorkers per year for the New York City Community Health Survey
("CHS"). It is anticipated that the selected vendor
("Contractor") will conduct both the Six Year Annual Community Health
Survey - 2015-2020 ("Annual Survey") and the CHS Heart Follow-up
Study Option in 2015 ("HFUS2").
