p.m. The deadlines for the full application is June 6, 2014 at 1 p.m.
Funding
Amount: Funding requests can range from $50,000 to
$400,000. Special Projects Fund grants are typically in the $250,000 range.
Eligibility: 501(c)(3)
and for-profit organizations with a significant presence in NYS. Applicants
must show that the proposed project further the charitable mission of the New
York State Health Foundation.

Summary: The New
York State Health Foundation (NYSHealth) has a broad mission to improve the
health of New York State residents. In order to have a tangible impact, most of
the Foundation's grantmaking is focused on three priority areas: expanding
health care coverage, improving diabetes prevention, and advancing primary
care. NYSHealth Special Projects Fund awards are one-time, nonrenewable
funding opportunities.
